Your official enrollment date is the date both this form and dues payment are received. For forms submitted in person, the enrollment date is the date of receipt. For forms submitted by mail, the postmark date will be used as the enrollment date, provided payment is received within a reasonable time. This date determines voting eligibility and facility rental discount eligibility. Please retain your enrollment confirmation.
